MAKE AN APPOINTMENT
We recommend that you call and make an appointment prior to coming to the health centers. We do our best to accommodate walk-ins and same-day, but cannot always do so. If you would like to make a same-day appointment, we recommend that you call before 9:00AM.

QueensCare Health Centers is a contracted (Medical Home) clinic partner with Health Care LA, IPA (HCLA). Through our partnership, we are able to care for patients enrolled with government sponsored managed care programs in Medi-Cal, Medicare Advantage and Medi-Medi (dual eligibles).

BENEFIT ENROLLMENT ASSISTANCE
There are a number of programs available to help low-income individuals and families pay for their healthcare costs.

QueensCare Health Centers′ multilingual Health Advocates and Eligibility workers are here to help new and existing patients in the community understand what health insurance programs exist, how they work and what they may qualify for.

Once in the clinic, ourCertified Application Assistants or Enrollers help patients complete the applications and follow up to ensure the patients are covered for the medical care they need. Follow up includes working with Medi-Cal social workers, choosing an HMO and helping families and individuals understand what the programs they qualify for cover.

All new QueensCare Health Centers patients will go through a benefit screening process during their first appointment.

GENERAL PAYMENT POLICIES
QueensCare Health Centers will bill your insurance provider for Professional Service Fees provided during the course of your visit. It is our policy to collect all co-pays, deductibles and fees not covered by your insurance at the time of visit. If you do not have insurance, our Certified Application Assistants will screen you to see if you qualify for a health plan. If you do, they will assist you in enrolling in the plan that best meets your family’s needs.

We accept Cash, MasterCard, Visa and Discover cards as means of payment.

A sliding fee scale is available to those who do not qualify for any program and do not have private insurance.

REQUEST TO CHANGE PROVIDER
To request a change in your primary care provider, please fill out the change form provided below and mail to your current home health center. A QHC representative will contact you within 48 hours AFTER receiving your request.

OBTAINING MEDICAL RECORDS
California law (AB610) allows the healthcare provider a 15-day processing period from the date a medical record request is received. QueensCare Health Centers’ turn-around time is about 15 business days.
