QueensCare Health Centers is just one of the many community clinics that utilize a disease management program to educate and empower patients with chronic diseases. This program is based on the Chronic Care Model (CCM) espoused by L.A. County Department of Health Services and the Community Clinics Association of L.A. County. Patient data for key measures are entered into the Patient Electronic Care System (PECS). PECS is a database for tracking and reporting quality of care and is used by clinics involved in health collaboratives of the Health Disparities Network such as the CALIFORNIA PRIMARY CARE ASSOCIATION>>.
Chronic diseases such as diabetes, hypertension and asthma can be influenced by lifestyle behaviors such as a healthier diet and increased physical activity. The Disease Management team of providers, care coordinators and clinical pharmacists lend that extra support to patients in the care of their chronic disease. Care coordinators are present in all the clinics to provide classes and one-on-one patient counseling.
