QueensCare Health Centers Foundation

Bringing Care to the Community Who Needs It Most

Body copy: The QueensCare Health Centers Foundation was developed to further enhance and enable QueensCare Health Centers’ (QHC) mission of providing care regardless of a patient’s ability to pay for services. Your financial support will help to ensure that mission continues for years to come.

Subhead: Our Patient Community

Body copy: Our patient population is primarily low-income households, largely Latino, working poor, and mostly immigrant populations from areas surrounding downtown Los Angeles. According to QHC”s 2021 UDS report, nearly 99% of patients live at 200% below the Federal Poverty Guideline; 90% are Medi-Cal recipients; 31% are uninsured.

Our Purpose

QueensCare Health Centers is one of the leading community health centers in Los Angeles County. QHC’s focus is on health promotion, disease prevention, health education and empowerment in conjunction with primary care. QueensCare Health Centers invests in health as opposed to illness, is a visible and trusted part of the Los Angeles community, and its care model addresses the most urgent health issues plaguing our communities.

QHC has a long and rich history of caring for the Los Angeles community, spanning more than a century. The organization understands that communities and organizations must identify and build on existing assets to advance positive healthcare and social change. QHC’s healthcare model fosters pathways to positive health outcomes through excellence in care, compassionate staff, and trusted community partnerships.

To effectively reduce disparities in primary care, WHC believes that patient-centered care must be delivered with compassion, dignity, and kindness. Our services include adult and pediatric medicine, dentistry, optometry, women’s health, integrated behavioral health, and specialty medical services.

Include quote from Eloisa Perard: “QueensCare Health Centers is an essential provider of primary healthcare services to low-income individuals and families in Los Angeles County. Many of our patients cannot afford care on their own and would go without if not for our approach of caring for patients regardless of their ability to pay. Support from community partners like you, help to advance our mission and care for those most in need.” – Eloisa Perard, President and CEO

Our Reach
Each year we serve over 23,000 patients across five clinic locations in some of the most underserved neighborhoods in Los Angeles

• Echo Park

QHC’s health centers serve an important clinical need, without our care, many patients would see their care at local hospital emergency rooms, further impacting an already overtaxed system.

Where We Receive Our Funding

As a Federally Qualified Health Center, QHC receives funding from the federal government, but these funds do not cover all operating expenses. If not for philanthropy, QHC would not be able to provide the quality level of care available at our health centers.

Financial support from our diverse group of donors – including individuals, foundations, government and corporate partners, and others – helps us to cover many essential services so our patients can receive exceptional care no matter their circumstances. Our donors can very easily see the value their contributions provide and the impact these investments make.

Your Support Furthers Our Efforts

Subhead/make stand out: QueensCare Health Centers is a special place of healing and care. Our healthcare providers, staff, donors, patients, and friends all contribute to making QHC extraordinary.

Please join us in supporting this community of care givers as we work toward eliminating healthcare disparities and work every day for improved health for all. We are proud of our positive impact our care provides to the community and know we can do more with your support. We would be grateful for your gift of any size to support our mission-driven work.

Our Need

This year, we have set an aggressive but necessary fundraising goal of $500,000 to cover essential investments in people, programs, and services that are otherwise not covered by patient care and government support – the medically uninsured and underinsured. The key areas of investment for the $500,000 in this fiscal year include:

Access to Care – $250,000

Mental & Integrated Behavioral Health – $200,000

Care Management: Obesity & Diabetes – $50,000
Patient Pull Quote: “I have been a patient at QHC for 14 year. In this time, I have accessed all the services offered, from primary care, to dental and vision, and even specialty care. Being able to have a health center in my neighborhood means I am able to receive care when I need it. And I know I live a healthier and fuller life because of QHC.” – Manuel Romero, Patient and Board Member

Thank you for your thoughtful consideration of QueensCare Health Centers as an organization to support with your philanthropic resources.

Please know that we treasure your investment and partnership.
